These patients had diabetes mellitus, advanced proteinuria, and hypertension, and renal pathologic findings on autopsy demonstrated glomerulosclerosis. It is important to recognize that diabetic patients also can have nondiabetic renal disease, such as glomerulonephritis and tubulointerstitial nephritis. Care of these patients is best considered as a team of clinical providers, educators, nurses, and nutritionists. For the purpose of the classification, at least one convincing KimmelstielWilson lesion (as in panel [f]) needs to be present. This has been largely attributed to a better glycemic control in diabetic patients. This combination has been shown to have an additive effect in reducing proteinuria in some studies.
